VNA Care Network & Hospice Is Pleased to Announce the Medication Home Visit Program
With funding from HESSCO Elder Services and the Massachusetts Executive Office of Elder Affairs with funds from Title III of the Older American’s Act.
A nurse will visit chronically ill, frail seniors at home to:
Evaluate medications
Offer education about medications
Assess home safety
Provide referrals to physicians, agencies and community resources
Free. Open to seniors age 60 and older who meet program criteria and live in towns served by HESSCO Elder Services: Canton, Dedham, Foxborough, Medfield, Millis, Norfolk, Norwood, Plainville, Sharon, Walpole, Westwood and Wrentham.
